Enforcement discretion for 503A pharmacies ended March 5, 2025 (after a court denied a preliminary injunction; FDA's earlier stated date was February 18, 2025) and for 503B outsourcing facilities on March 19, 2025. Since then 503A compounding is limited to patient-specific preparations that are not essentially copies of approved products, and FDA's April 1, 2026 reminder and a September 18, 2026 Warning Letter (tirzepatide/niacinamide products) show active scrutiny of added-ingredient versions made at volume. On April 30, 2026 FDA proposed excluding tirzepatide (with semaglutide and liraglutide) from the 503B bulks list; a final determination had not been confirmed as of AffinityRx's October 2026 review.
