Semaglutide is the active ingredient of FDA-approved drugs, but FDA determined the semaglutide injection shortage resolved on February 21, 2025.

FDA's enforcement discretion for 503A pharmacies ended April 22, 2025 (confirmed ended after a court denied a preliminary injunction on April 24, 2025), and for 503B outsourcing facilities on May 22, 2025. Since then, a 503A pharmacy may compound only a patient-specific preparation that is not essentially a copy of an approved product (a prescriber-documented clinically significant difference), and FDA's April 1, 2026 notice and a September 18, 2026 Warning Letter to a 503A pharmacy treat added-ingredient products produced at volume as potentially pretextual copies. On April 30, 2026 FDA proposed excluding semaglutide, tirzepatide and liraglutide from the 503B bulks list for lack of clinical need (comment period extended; a final determination had not been confirmed as of AffinityRx's October 2026 review). FDA states it knows of no lawful basis to use semaglutide salt forms (semaglutide sodium, semaglutide acetate) in compounding.
