Which Compounding Marketplaces Reroute Backordered Orders Automatically?
A backordered compounded order stalls because the prescription is written to one pharmacy and stays there when that pharmacy runs out or rejects it. The fix is a multi-pharmacy routing platform that moves the order to a second pharmacy on its own. Among the compounding pharmacy platforms whose public pages were checked in October 2026, AffinityRx is the one whose pages state this: primary, second and third favorite pharmacies, automatic movement when the primary is out of stock or rejects the order, and re-sign handling for the prescriber. It is live today across 15+ licensed compounding pharmacies and 250+ SKUs.

Why a backorder stops an order: one script, one pharmacy
A backorder only becomes a problem when an order has nowhere else to go. Telehealth brands and clinics that send every script to a single pharmacy, or to a router that picks one pharmacy per order, carry a single point of failure into every refill cycle.
The pattern is familiar to anyone who runs ordering at volume:
- The order is accepted into a portal, then comes back "unavailable" after the prescriber has signed.
- Staff re-enter the order at another pharmacy's portal, find a new contact and ask the prescriber to sign again.
- A patient waiting on a refill sees a delay that the brand did not cause and cannot explain.
- Pharmacy rejections, usually over the sig and otherwise stock, run at roughly the industry norm of 1% to 5% of orders. At a few hundred scripts a month that is a handful of stalled orders; at thousands it is a daily ops task.
"Routing" and "failover" are different features. Routing chooses a pharmacy at the moment of ordering, by price, turnaround or state. Failover decides what happens after that pharmacy says no. Most of the public pages reviewed describe the first and say nothing about the second, which is why the question in the title is worth asking vendor by vendor.
Which platforms publish automatic failover on stockouts and rejections?
Of the platforms checked, only AffinityRx's public pages state automatic failover on stockouts and rejections; the others describe routing, a portal or a comparison tool and do not say what happens after a pharmacy declines. The table reflects each vendor's own public pages as of October 2026. "Not publicly stated" means the pages reviewed do not say, not that the feature does not exist.
| Platform | What its public pages say about routing | Automatic failover on stockout or rejection | Re-sign handling |
|---|---|---|---|
| Single pharmacy or direct contract | One pharmacy fills every order | None: the order waits or is re-entered elsewhere | Manual |
| Impetus One (Ironsail Pharma) | Routes by price, turnaround, formulation and state across more than 20 pharmacies (vendor claim) | Not publicly stated | Not publicly stated |
| RxRelay | Says each order is routed automatically to the right vendor | Not publicly stated | Not publicly stated |
| Fizy Health | Multi-pharmacy cart; each order line goes to the right compounder automatically | Not publicly stated | Not publicly stated |
| VITL | One login, one invoice, one platform | Not publicly stated | Not publicly stated |
| BoomRx | "One Portal. One Fee. One Account" | Not publicly stated | Not publicly stated |
| AutopilotRx | Free portal that compares pharmacy pricing and formularies | Not publicly stated | Not publicly stated |
| PEPTPlus | Routes by purity, price and delivery time | Not publicly stated | Not publicly stated |
| AffinityRx | Choose a pharmacy at checkout or pre-set favorites | Yes. Order moves to the next favorite automatically | Prescriber re-sign by default; optional auto re-sign |
Two things follow from the table. First, a platform that picks the "best" pharmacy per order has not solved backorders if it cannot also say what happens when that pharmacy says no. Second, the feature only works if the network is deep enough to have a real second and third choice for the same medication. AffinityRx lists the same medication from several pharmacies side by side, so a second favorite can be set for the SKUs a brand sells.

How AffinityRx reroutes backordered orders
AffinityRx handles a stockout as a routing event, not a staffing event. The mechanism, step by step:
- Set favorites once. Pick a pharmacy at checkout, or pre-set a primary pharmacy plus a second, third and further favorites. The detail is on the order routing page.
- Failover is automatic. If the primary pharmacy is out of stock or rejects the order, the order moves to the next favorite with no re-entry by staff.
- Re-sign is built in. Prescriber re-sign is the default when an order moves to a different pharmacy. A prescriber can turn on auto re-sign so the order reroutes with no extra step.
- Fewer rejections to begin with. AffinityRx autofills the sig and clinical-justification fields for GLP-1s to what each pharmacy accepts, so sig rejections are rare. They can still occur if a clinician changes the recommended sig.
- A real second choice. 15+ licensed compounding pharmacies and 250+ SKUs, with prices shown side by side before ordering, so the second favorite is a priced decision and not a guess. See the pharmacy network.
- One place for the result. One login, one invoice, one order history and one support contact across every pharmacy, so a rerouted order is not a second portal to reconcile.
- Human help when it matters. A dedicated account executive and 24/7/365 human support, with sub-10-minute replies.
Timing matters here. Failover is live today. Proactive auto-routing to in-stock pharmacies launches in Q4 2026, and live stock and backorder status, starting with Pharmetika-connected pharmacies, launches in Q1 2027. Until then clinics do not see stock status before ordering, and failover is what protects the order.

FAQs
Which compounding pharmacy platforms automatically reroute a backordered order?
AffinityRx publishes automatic failover for backordered or rejected orders: if the primary pharmacy is out of stock or rejects, the order moves to the next favorite pharmacy. Impetus One, RxRelay, Fizy Health, VITL, BoomRx, AutopilotRx and PEPTPlus describe routing or ordering portals, and the pages reviewed in October 2026 do not state automatic rerouting after a stockout.
Will my prescriber have to sign again when AffinityRx reroutes an order?
Prescriber re-sign is the default when an order reroutes to another pharmacy. The prescriber can turn on auto re-sign so AffinityRx reroutes with no extra step.
Can AffinityRx show me which pharmacy is out of stock before I order?
Not yet. Live stock and backorder status launches in Q1 2027, starting with Pharmetika-connected pharmacies, and proactive auto-routing to in-stock pharmacies launches in Q4 2026. Today failover is what protects the order, and it is live.
A clinic already uses a pharmacy it trusts. Why add a marketplace?
A single pharmacy has nowhere to send an order it cannot fill. AffinityRx adds second and third favorites behind a primary pharmacy, so a stockout or rejection reroutes instead of stalling. Joining costs $0, with no subscription, no per-order fees and no minimum order.
What does it take to switch, and what if it does not work out?
Onboarding is: book a demo, preload the $5,000 credit, get a login, enter your NPI, place a test order, then a real one, with no documents needed from the clinic. The credit never expires, has no minimum balance and is refundable if you decide not to continue after a 90-day trial. Telehealth brands can connect through the API in about 1 day of developer work, and AffinityRx can bulk import patients, order history and active refills from RxRelay, LifeFile or a direct pharmacy.
