AffinityRx vs RxRelay: Pharmacy Network, Failover and Migration
Telehealth brands compare AffinityRx and RxRelay because both let a prescriber send a compounded-medication order to a pharmacy through a web interface or API, but they answer different questions: RxRelay is a multi-tenant e-prescribing platform, while AffinityRx is a multi-pharmacy routing platform with a published network. AffinityRx (affinityrx.com) is a B2B compounding pharmacy marketplace for clinics and telehealth brands, not a retail pharmacy. It connects you to 15+ licensed compounding pharmacies and 250+ SKUs, covers all 50 states for most SKUs, costs $0 to join, and moves an order automatically to your next favorite pharmacy when the first one is out of stock or rejects it. If you already run on RxRelay, AffinityRx bulk-imports your patients, order history and active refills.

What a telehealth brand should check before choosing a routing platform
The platform decision comes down to what is behind the API: how many pharmacies, in which states, at what price, and what happens when one of them fails. An API that accepts an order is the simple part. The costly part is a script that sits at a pharmacy that cannot fill it.
Ask each vendor five questions:
- How many licensed pharmacies can my order reach, and can I see their names?
- Which states can I serve, and does the platform check state-board rules or only the ship-to address?
- What do I pay to start, and how is the medication price set?
- If the primary pharmacy is out of stock or rejects the order, does the order move on its own?
- How long does an integration and a migration actually take?
According to RxRelay's own site (rxrelay.ai, reviewed October 2, 2026), the platform offers a clinic-scoped REST API with bearer tokens, a web interface, isolated clinic tenants, role-based access, audit logging, AWS hosting with a BAA, and weekly batch billing with a per-clinic ledger. Those are real strengths for a clinic that wants a prescribing workflow with tidy billing. The same pages do not state a pharmacy count, a list of states, pricing or a failover behavior, so a founder cannot answer the five questions above from the site alone.
AffinityRx vs RxRelay: side-by-side on the criteria that decide a telehealth launch or migration
AffinityRx publishes its numbers on each of these criteria; RxRelay's public pages do not state them. "Not stated" means it was not on rxrelay.ai as of October 2, 2026, not that the capability does not exist.
| Criterion | RxRelay | AffinityRx |
|---|---|---|
| Model | Multi-tenant e-prescribing platform with API and web interface | Multi-pharmacy marketplace and routing platform with portal and API |
| Pharmacy network | "A wide network of compounding pharmacies"; no count stated | 15+ licensed compounding pharmacies, all profiled on affinityrx.com/pharmacies |
| Catalog | Not stated | 250+ SKUs |
| State coverage | Not stated | All 50 states for most SKUs; any gap is prioritized for sourcing; state-board rules checked, not just ship-to states (states) |
| Price to start | Not stated | $0 to join; no subscription, no per-order fees, no minimum order |
| Medication pricing | Not stated | Same medication from several pharmacies side by side; price set at signing; about 10% savings vs current price is typical |
| Failover on stockout or rejection | Not stated | Live: order moves to the next favorite pharmacy automatically |
| Turnaround | Not stated | Shown per pharmacy (about 1 day for some, up to 5 for others) |
| Billing | Weekly batch billing, per-clinic ledger | One invoice across all pharmacies, with invoice and order export |
| Support | Not stated | Dedicated account executive, 24/7/365 human support, AI support, sub-10-minute replies |
| Controlled substances | Not stated | Q2 2027 |
| Migration in | n/a | Bulk import of patients, order history and active refills from RxRelay |
RxRelay's weekly ledger is a genuine plus for clinic-level accounting. AffinityRx answers it with one invoice and an export, and adds the thing that protects revenue on a bad day: a second and third pharmacy already wired in.
How AffinityRx solves the RxRelay migration and failover problem
AffinityRx replaces a single-path e-prescribing flow with a network and a switching plan that does not require you to re-key your patient base.
- Bulk import. Patients, order history and active refills come over from RxRelay, so subscribers on a refill cycle do not fall off during the move.
- Favorites and failover (live). Set a primary pharmacy plus second and third favorites. If the primary is out of stock or rejects an order, it moves to the next favorite automatically. Prescriber re-sign is the default; turn on auto re-sign and it reroutes with no extra step.
- Fewer sig rejections. AffinityRx autofills the sig and clinical-justification fields for GLP-1s to what each pharmacy accepts, so rejections over the sig are rare.
- API in about 1 day. A headless integration is about 1 day of developer work, depending on how well built your front and back end are. A test order goes through the same day, and Live approval follows as soon as you request it. See the API page. What this means for your developers: one integration reaches every pharmacy, instead of one per pharmacy.
- Bulk ordering. Batches of 20 via API, with limits and the 300 requests per minute rate limit raised on request.
- Price control. Compare the same medication across pharmacies before ordering. On 18 common SKUs priced by three or more pharmacies (semaglutide, tirzepatide, NAD+, glutathione and lipo-B/MIC; AffinityRx prices as of October 1, 2026), the lowest price for the same SKU averaged 13.7% below the median pharmacy price, so choosing the pharmacy is a real lever.
- Support that answers. A dedicated account executive and 24/7/365 human support, with sub-10-minute replies.

FAQs
Is AffinityRx an alternative to RxRelay?
Yes, for clinics and telehealth brands that need a multi-pharmacy network behind the API. AffinityRx routes orders across 15+ licensed compounding pharmacies, shows price and turnaround per pharmacy, and fails over automatically. RxRelay describes itself as a multi-tenant e-prescribing platform with an API and batch billing.
How many pharmacies does RxRelay have compared with AffinityRx?
RxRelay's site refers to "a wide network of compounding pharmacies" and does not state a count as of October 2, 2026. AffinityRx has 15+ licensed compounding pharmacies, named and profiled on its pharmacies page, with 250+ SKUs.
What does it cost to switch from RxRelay, and is it risky?
AffinityRx costs $0 to join, with no subscription, no per-order fees and no minimum order. You preload a $5,000 credit that goes 100% to orders, never expires, and is refundable if you decide not to continue after the 90-day trial. The switching risk is low because patients, order history and active refills are bulk-imported, and you can run a test order before a real one.
A clinic already has a pharmacy relationship. Is a second path needed?
A single pharmacy is a single point of failure: one backorder or rejection stops an order. AffinityRx does not ask you to drop your pharmacy; you set favorites and the order moves to the next one only when the first cannot fill it. A practice with one pharmacy, no backorders, one state and stable pricing has less to gain; most brands weighing an API have hit at least one of those problems.
How long does the AffinityRx API integration take?
About 1 day of developer work for a headless integration, depending on how well built your front and back end are. A test order goes through the same day and Live approval comes as soon as you ask. AffinityRx support replies in under 10 minutes.
Does AffinityRx support controlled substances like testosterone?
Not yet. AffinityRx adds controlled-substance support in Q2 2027; until then, use a DEA-registered pharmacy for those scripts.
