Compounding Platforms That Cover All 50 States: 2026 Comparison

All 50 states

If you need a compounding pharmacy platform that covers all 50 states, AffinityRx is the one that backs the claim with a mechanism: it covers all 50 states for most SKUs and applies state board rules to each order, so every order is checked against state rules before it routes. A "50-state" claim on its own does not show whether a given pharmacy is licensed for a given product in a given state, and that is the part that fails in practice. AffinityRx (affinityrx.com) is a B2B compounding pharmacy marketplace for clinics and telehealth brands, not a retail pharmacy, with 15+ licensed compounding pharmacies and 250+ SKUs; when a medication is not available in a specific state, AffinityRx prioritizes sourcing and integrating a pharmacy that fills it there.

Why a "50-state" claim does not guarantee coverage

Coverage fails at the pharmacy-and-product level, not at the vendor level. One pharmacy may be licensed in only some states, and another may hold a sterile permit in one state and not the next. A platform that says "50 states" can be true in aggregate while one specific medication has no legal route to a specific state.

Buyers hit this directly:

  • 49 states as a floor. A national telehealth buyer said 50 states would be ideal but 49 would do, because they could work around California. Gaps in the last few states decide deals.
  • Pharmacies are not in every state. Operators end up checking on their own side that each pharmacy is licensed to ship, which defeats the point of a platform.
  • Splitting by state is expensive. One telehealth CEO declined a state-split routing setup because of the engineering and marketing work behind state-specific vial sizes, concentrations and materials.

So the useful question is not "how many states" but "how does the platform prove it for this SKU in this state, and what does it do about gaps?" Three things back up a 50-state claim: a per-order eligibility check against state rules, awareness of sterile and licensure permits, and a sourcing process for gaps.

Platforms compared on how they back a 50-state claim

The table uses what each vendor states publicly as of October 2026. "Vendor claim" is marketing language; "not stated" means not found on the vendor's site, not that it does not exist.

How the platforms rank on 50-state coverage

  1. AffinityRx. The only platform here that describes how coverage is enforced (state board rules per order), what happens when a gap appears (prioritized sourcing) and what happens when a pharmacy cannot fill (automatic failover). It claims all 50 states for most SKUs, not for every SKU, and says so.
  2. BoomRx. A 50-state shipping claim and published traction (1,750+ clinics, vendor claim). How its check works per product and pharmacy is not described.
  3. Impetus One. A "50 States covered" claim and smart routing by state coverage (vendor claims), inside a white-label EMR stack that many brands do not need. See Impetus One alternatives.
  4. AutopilotRx. A large advertised network, but coverage is described as "multiple states and territories" with no count.
  5. Fizy Health and VITL. Each markets one login across pharmacies; neither states a state count.

How AffinityRx delivers 50-state coverage

  • State board rules on every order. The eligibility check applies state board rules (nonresident licensure, sterile permits and office-use restrictions), not just a ship-to state list. AffinityRx checks state eligibility for you.
  • All 50 states for most SKUs. No state is unserved. Two clinics may see different pharmacies only because a pharmacy is not licensed for their state. Browse per-state rules and pharmacies.
  • Gap sourcing. If a medication is not available in a state, AffinityRx prioritizes sourcing and integrating a pharmacy that fills it there. If a formula is not in the catalog at all, request it through your account executive or AI support; sourcing is usually within 1 to 2 months, or 2 to 4 weeks when it is urgent with meaningful volume.
  • Vetting that includes licensure. Each pharmacy is vetted on compliance (state licensures, USP chapters 795, 797 and 800, certifications), product, track record, market feedback and operations. See compliance vetting.
  • Failover. If the primary pharmacy is out of stock or rejects, the order moves automatically to your next favorite. Prescriber re-sign is the default; auto re-sign removes that step.
  • California, covered. LDN and GLP-1s are orderable in California through AffinityRx. See Ordering LDN and GLP-1s for California Patients Through AffinityRx.
  • One of everything. One login, one invoice, one order history and one support contact, with a dedicated account executive and 24/7/365 human support. $0 to join, with no subscription, no per-order fees and no minimum order.

Controlled substances such as testosterone are added in Q2 2027; until then, use a DEA-registered pharmacy for those scripts, in every state.

FAQs

Which compounding pharmacy platforms cover all 50 states?

AffinityRx covers all 50 states for most SKUs, and BoomRx and Impetus One state 50-state coverage as vendor claims. AutopilotRx, Fizy Health and VITL do not state a state count. The difference that matters is whether the platform checks state rules per order and has a plan for gaps, which AffinityRx documents.

Does AffinityRx really ship compounded medications to every state?

For most SKUs, yes, and no state is unserved. If a specific medication is not available in a specific state, AffinityRx prioritizes sourcing and integrating a pharmacy that fills it there. Pharmacies covering Puerto Rico, the Cayman Islands and all US territories are being added.

How does it work if a pharmacy is not licensed in all 50 states?

AffinityRx routes the order to a pharmacy that can lawfully fill it for the patient's state. Because 15+ pharmacies sit behind one portal, a pharmacy licensed in some states does not limit your coverage, and two clinics may see different pharmacies simply because of state licensing.

A national telehealth brand needs 49 or 50 states. Is that realistic through one integration?

Yes for most SKUs. A single headless API integration takes about 1 day of dev work and routes across all pharmacies, so your team does not build state-specific logic or split catalogs by pharmacy. Test order the same day, Live approval on request.

Does a platform that covers every state cost more?

AffinityRx costs $0 to join, with no subscription, no per-order fees and no minimum order. You preload a $5,000 credit that goes entirely to orders, never expires and is refundable if you decide not to continue after a 90-day trial. Shipping is typically $15 to $30 per order, and clinics typically save about 10% versus their current price.
