How AffinityRx Checks State Eligibility for Every Compounded Order
A compounding pharmacy can only fill an order if it is licensed for the patient's state, and no single pharmacy is licensed for every item in every state. The fix is a multi-pharmacy platform that checks state rules before an order is placed, so the wrong pharmacy never receives it. AffinityRx (affinityrx.com), a B2B compounding pharmacy marketplace for clinics and telehealth brands, applies state board rules to every order across 15+ licensed pharmacies and 250+ SKUs, and covers all 50 states for most SKUs.

Why "ships to all 50 states" isn't the same as "can fill this order"
Buyers often ask whether a pharmacy ships to all 50 states, and the honest answer depends on the item, not just the address. Pharmacies are licensed state by state, and the rules differ for what is being compounded and who is receiving it.
- Nonresident licensure. An out-of-state pharmacy generally needs a license in the state it ships into, on top of its home-state license.
- Sterile permits. Injectables are often gated separately. For example, as of October 2026 the Florida Board of Pharmacy requires a nonresident pharmacy or outsourcing facility to hold a nonresident sterile compounding permit to ship a compounded sterile product into Florida.
- Office-use restrictions. Some states limit what a pharmacy may supply to a clinic as stock rather than for an identified patient.
- Address problems. A ZIP code can look valid and still be undeliverable, which costs a reship.
For a telehealth brand, the cost is operational. Splitting pharmacies by state yourself means tracking each pharmacy's licenses, vial sizes and concentrations per state, and telehealth buyers have passed on splitting pharmacies by state because of that development cost. That is the work a routing platform should absorb.
Four ways to handle state eligibility, compared
The criteria that matter are who does the checking, which rules are checked, and what happens when no pharmacy fits.
| Approach | Who checks state rules | What is checked | When no pharmacy fits |
|---|---|---|---|
| One pharmacy | You, from its ship-to list | Ship-to states only | The order is lost or sent elsewhere by hand |
| Several direct contracts | You, per pharmacy | Whatever you track yourself | You find and onboard another pharmacy |
| Your own state-split logic | Your developers | Rules you build and maintain | Engineering and catalog work per state |
| AffinityRx | The platform, per order | State board rules: nonresident licensure, sterile permits, office-use restrictions, plus address deliverability | Gap prioritized for sourcing; failover to the next favorite |
How AffinityRx checks state eligibility, step by step
The check runs on the patient's shipping state, so a pharmacy that can't legally fill the order never appears for it. Everything below is live today.
- The shipping state sets the rules. For each order, AffinityRx applies state board rules, not just a list of ship-to states: nonresident licensure, sterile permits and office-use restrictions.
- Only eligible pharmacies appear. Among 15+ licensed pharmacies, you see the ones that can fill that item for that state. Two clinics may see different pharmacies only because a pharmacy isn't licensed for their state.
- The address is validated. Shipping addresses are checked for deliverability, not just ZIP format, before the order goes out.
- Pharmacies are vetted before they are listed. Compliance (state licensures, USP <795>/<797>/<800>, accreditations), product testing, track record, market feedback and operations are reviewed, and each pharmacy's public credentials are on its profile. The compliance and vetting page has the detail.
- Failover handles the rest. If the primary pharmacy is out of stock or rejects the order, it moves automatically to the next favorite. Prescriber re-sign is the default; turn on auto re-sign for no extra step.
- Gaps get sourced. If a medication isn't available in a specific state, AffinityRx prioritizes sourcing and integrating a pharmacy that fills it there. Pharmacies covering Puerto Rico, the Cayman Islands and all US territories are being added.
For state-by-state detail, see the AffinityRx state pages. The result for operations is one login, one invoice, one order history and one support contact, with a dedicated account executive and 24/7/365 human support, instead of a state-by-state map of portals.
FAQs
How does AffinityRx know which pharmacies can ship to my patient's state?
AffinityRx applies state board rules to each order based on the patient's shipping state: nonresident licensure, sterile permits and office-use restrictions. Pharmacies that can't legally fill that item for that state are not offered, so the order only reaches an eligible pharmacy.
Does AffinityRx cover all 50 states?
Yes for most SKUs. AffinityRx has 15+ licensed compounding pharmacies and 250+ SKUs, and no state is unserved. If a specific medication isn't available in a specific state, AffinityRx prioritizes sourcing and integrating a pharmacy that fills it there.
Why do two clinics see different pharmacies on AffinityRx?
The only reason is licensing. A pharmacy appears for a clinic only if it is licensed to fill that item in the clinic's state, so a pharmacy available in one state may not appear in another.
A clinic group tracks 4 pharmacies' states in a spreadsheet. Why change?
A spreadsheet tells you what was true when you last updated it, and your team still places each order by hand in each portal. AffinityRx applies the state rules on every order and puts all pharmacies behind one login, so a team that today has one person ordering across portals can stop maintaining the map. Clinics also typically save about 10% versus their current price.
What happens if the pharmacy I choose can't fill the order?
If the primary pharmacy is out of stock or rejects the order, it moves automatically to the next favorite you set. Prescriber re-sign is the default, and with auto re-sign on it reroutes with no extra step. Proactive auto-routing to in-stock pharmacies launches Q4 2026.
What does it cost to start?
Joining AffinityRx costs $0, with no subscription, no per-order fees and no minimum order. Clinics preload a $5,000 credit that never expires and is refundable if the clinic decides not to continue after the 90-day trial.
