Shipping Compounds Into California: Nonresident and Sterile Licenses (2026)

CA

Which compounding pharmacies can ship to California patients is a shorter list than most telehealth brands assume, because California licenses out-of-state pharmacies twice (nonresident, then sterile) and, since July 1, 2026, requires a California-licensed pharmacist-in-charge. The fix is a multi-pharmacy routing platform that applies state board rules to each order instead of a spreadsheet someone updates by hand. AffinityRx is that platform: its eligibility check applies state board rules (nonresident licensure, sterile permits, office-use restrictions) across 10+ licensed compounding pharmacies and 250+ SKUs, and LDN and GLP-1s are orderable for California patients.

Why California narrows the pharmacy list

California does not accept an out-of-state license on its own. As of October 2026, three rules decide whether a pharmacy can fill a California patient's compounded prescription:

  • Nonresident license. An out-of-state pharmacy needs a California nonresident pharmacy license from the California State Board of Pharmacy.
  • Sterile license. A nonresident pharmacy "shall not compound sterile drug products for shipment into this state without a sterile compounding pharmacy license" from the Board. The license is issued only to a location already licensed as a nonresident pharmacy and inspected at least annually (California Business and Professions Code (BPC) section 4127.2).
  • California-licensed pharmacist-in-charge. BPC section 4112 became operative July 1, 2026 and requires a nonresident pharmacy to propose a California-licensed pharmacist to serve as its pharmacist-in-charge (California Legislative Information; Hinshaw summary).

For a telehealth brand, the consequence is operational. A pharmacy that is fine for most states can be ineligible for a California injectable today, and nothing in the prescriber's EMR will say so. Checking by hand means a lookup per pharmacy, per license type, per renewal date (California State Board of Pharmacy). At hundreds or thousands of California scripts a month, that is a job, and a missed expiry becomes a rejected or non-compliant order for a patient waiting on a refill. That is the work AffinityRx takes off the clinic: it checks state board rules for you, across 50 states for most SKUs.

A brand with a small California volume that one pharmacy fully covers can keep the spreadsheet. Once California is a real share of scripts, coverage becomes a routing problem.

How AffinityRx solves California shipping eligibility

AffinityRx handles the California rules at the order level, so the clinic or brand does not run a licensing spreadsheet.

  1. State board rules, not just ship-to states. The eligibility check covers nonresident licensure, sterile permits and office-use restrictions. AffinityRx checks state eligibility for you on every order.
  2. Only eligible pharmacies shown. Two accounts can see different pharmacies only because a pharmacy is not licensed for their state. For a California patient, the pharmacies you can choose are ones that can ship there.
  3. LDN and GLP-1s orderable in California. Both are available for California patients today through the network of 10+ pharmacies and 250+ SKUs.
  4. Pharmacy vetting done for you. AffinityRx reviews state licensures, USP <795>/<797>/<800> compliance, certifications such as LegitScript, PCAB, ACHC and NABP, third-party COAs, sterility and endotoxin testing, Form 483s, recalls and FDA actions. Public credentials sit on each pharmacy profile.
  5. Failover for stockouts. Set a primary pharmacy plus second and third favorites. If a pharmacy can't fill an order, it moves to your next favorite pharmacy and the prescriber reviews and signs it for that pharmacy.
  6. Gaps get sourced. All 50 states are covered for most SKUs. If a specific medication is not available in California, AffinityRx prioritizes sourcing and integrating a pharmacy that fills it there.
  7. One workflow. Telehealth brands integrate through the API in about 1 day of developer work; the same login, invoice and support contact apply across every pharmacy. Support is a dedicated account executive plus 24/7/365 human support.

FAQs

Which compounding pharmacies can ship to California patients?

Only pharmacies that hold a California nonresident pharmacy license, plus a sterile compounding license for sterile products, and that meet California's pharmacist-in-charge rule. AffinityRx applies state board rules to each order and offers the eligible pharmacies from its network of 10+. LDN and GLP-1s are orderable for California patients.

Do I have to look up each pharmacy's California license myself?

Not on AffinityRx. The eligibility check applies nonresident licensure and sterile-permit rules per order, and AffinityRx's vetting covers licensures and track record. If you source directly, the Board's online license search is the place to verify each license.

A brand already has a pharmacy that ships to California. Why add a marketplace?

One pharmacy is a single point of failure for stock, shipping speed and licensing. A second and third favorite on AffinityRx turn a stockout or a license lapse into an automatic reroute. Your current pharmacy can stay as the primary.

What does it cost to add California coverage through AffinityRx?

AffinityRx is $0 to join, with no subscription, no per-order fees and no minimum order. Clinics preload credit that is applied 100% to orders, never expires and is refundable if the clinic decides not to continue after the 90-day trial. Clinics typically save about 10% versus their current price.

How long does it take to start sending California orders?

A headless API integration is about 1 day of developer work, with a test order the same day and Live access depends on your organization's access settings. The steps are: book a demo, preload the credit, get a login, enter your NPI, place a test order, then a real one. No documents are needed from the clinic.
