Your GLP-1 Pharmacy Dropped a Formulation: Keep Patients on Track
When a pharmacy stops making a compounded GLP-1 formulation, every active refill tied to it stalls at the same moment, because the subscriber base is hard-wired to that one pharmacy. The fix is a multi-pharmacy routing platform where vetted alternates are already set as favorites and orders move on their own. AffinityRx is that router for telehealth brands: it lists several pharmacies per GLP-1 SKU across 10+ licensed compounding pharmacies and 250+ SKUs, and when the primary pharmacy cannot fill or rejects an order, AffinityRx moves it to the next favorite automatically.

Why a dropped formulation hits a telehealth brand harder than a stockout
A backorder is temporary and a discontinued formulation is not. If a pharmacy tells a telehealth brand it will no longer make a formulation, the brand does not get a "try again Thursday." It gets a permanent hole in a refill queue that may hold hundreds or thousands of scheduled orders.
Three problems show up at once:
- Concentration risk. Brands that route most GLP-1 volume to one pharmacy have no second source for that exact product.
- A rebuild under deadline. Finding, contracting and integrating a replacement pharmacy takes weeks when done one pharmacy at a time, and each one means another portal or API connection.
- A customer-facing gap. Subscribers do not care which pharmacy dropped what. They see a late shipment, and late shipments are where cancellations come from.
Dropped GLP-1 formulation: what each sourcing setup does
The table compares how four setups respond when the primary pharmacy cannot supply a formulation. A brand whose pharmacy has never changed its GLP-1 menu may not feel this risk yet; the rest of this article is for brands that have just felt it or want to be ready before they do.
| When the primary pharmacy drops a formulation | Single pharmacy | Direct contracts with 2 or 3 pharmacies | Single-pharmacy API or ordering portal | AffinityRx |
|---|---|---|---|---|
| Alternate already integrated | No | Only the ones you built and maintain | Only that vendor's pharmacies | Yes: several pharmacies per GLP-1 SKU in one account |
| Open refills on the dropped formulation | Stall until someone re-routes them | Re-keyed to the other pharmacy by staff | Depends on the vendor | Move to the next favorite when the primary rejects or cannot fill |
| Developer work to add an alternate | A new integration | A new integration per pharmacy | Depends on the vendor | None: same API, about 1 day of work in total |
| Compare formulations and prices first | No | One pharmacy at a time | Depends on the vendor | Same medication from several pharmacies side by side, prices shown before ordering |
| Operations footprint | 1 portal | 2 or 3 portals, invoices and contacts | 1 portal | 1 login, 1 invoice, 1 order history, 1 support contact |
| Support during the switch | Pharmacy rep | Each pharmacy's rep | Depends on the vendor | Dedicated account executive plus 24/7/365 human support |
How AffinityRx solves a dropped GLP-1 formulation
AffinityRx makes any one pharmacy replaceable, so a dropped formulation becomes a routing event instead of a rebuild. The mechanisms:
- Failover across favorites (live). Set a primary pharmacy plus second, third and further favorites. If the primary is out of stock or rejects the order, which is what a pharmacy that no longer makes the item does, the order moves to the next favorite with no ticket. See order routing.
- Alternates you chose in advance. Compare the same medication across pharmacies side by side, with price and turnaround (about 1 day for some pharmacies, up to 5 for others) shown before you order. See the tirzepatide page for options.
- Prescriber in the loop. The prescriber signs when an order changes pharmacy, so a prescriber reviews the switch. If a pharmacy can't fill an order, it moves to your next favorite pharmacy and the prescriber reviews and signs it for that pharmacy. Product and strength differences between pharmacies are a prescriber decision, not a platform one.
- No new integration per alternate. A headless API integration takes about 1 day of developer work, with a test order the same day. Refills are supported, and bulk ordering runs in batches of 20 via API, with limits raised on request.
- Fewer rejections. AffinityRx formats the prescriber's sig and clinical-justification entries to each pharmacy's required format. The prescriber writes or approves them and signs.
- Coverage across states. Most SKUs ship to all 50 states, and the eligibility check applies state board rules so a replacement is not one that cannot ship to your patients' states.
- Roadmap with dates. Proactive auto-routing to in-stock pharmacies launches Q4 2026, and live stock and backorder status launches Q1 2027. Today clinics do not see stock before ordering, so failover is what protects refills.
For a brand-level view, the telehealth weight-loss solutions page covers how GLP-1 programs source on AffinityRx. Brands typically save about 10% versus their current price; in one anonymized example, a weight-loss telehealth brand's per-vial compounded tirzepatide cost decreased after comparing pharmacies.
A short subscriber note for the switch

FAQs
What do I do if my pharmacy stops making a compounded GLP-1 formulation?
Move the affected orders to a second pharmacy that already carries the medication, and have the prescriber review the switch. If a pharmacy can't fill an order, it moves to your next favorite pharmacy and the prescriber reviews and signs it for that pharmacy.
How long does it take to switch GLP-1 pharmacies mid-treatment?
Adding an alternate on AffinityRx is not a new integration. A telehealth brand's API connection takes about 1 day of developer work, and a test order can go out the same day. Subscribers on existing refills can be bulk-imported from your current pharmacy system or a direct pharmacy, including order history and active refills.
Won't switching pharmacies change my price?
It can lower it, because pharmacy prices for the same product vary. For semaglutide and tirzepatide vials (13 SKUs with 3 or more pharmacies quoting, pricing as of October 1, 2026), the lowest-priced pharmacy averaged 9.8% below the median. AffinityRx shows prices side by side, and the price is set at signing.
How do I know a replacement pharmacy is any good?
AffinityRx vets each pharmacy on compliance (state licensure, USP standards, certifications such as LegitScript and PCAB), product (third-party certificates of analysis, sterility and endotoxin testing), track record (Form 483s, recalls, FDA actions), market feedback and operations. If a listed pharmacy receives an FDA warning letter or recall, clinics are notified and can switch that pharmacy off in their portal.
Can I see which pharmacy still has the formulation in stock?
Not yet. Live stock and backorder status launches Q1 2027, and proactive auto-routing to in-stock pharmacies launches Q4 2026. Until then, failover moves the order after a pharmacy reports it cannot fill.
A brand already has 2 pharmacies. Is a router still needed?
Brands usually look further when a third pharmacy is needed for coverage or price, and each added pharmacy means another portal or integration. AffinityRx replaces that with one API, one invoice and a dedicated account executive, with $0 to join and no minimum order.
